New details have emerged about the tragic circumstances surrounding the death of singer Naomi Judd.

Judd, the matriarch of the iconic country music duo with her daughter, was found unresponsive at her home after a suspected suicide in late April at the age of 76. An autopsy report revealed that she suffered from a gunshot wound and was found with a note containing “suicidal connotations,” Page Six reported.

“Per family, the decedent has had prior suicidal ideations and recent life stressors,” the autopsy report read, per the outlet. “A weapon and a note with suicidal connotations were found near the decedent at the scene.”

In May, Ashley Judd, her daughter, revealed during an interview with ABC that she was the one who found her mother dead and confirmed that her mother had killed herself. She said that her mother “used a firearm,” something that has since been corroborated by the autopsy report.

“She had an apparent self-inflicted gunshot wound and was transported to Williamson Medical Center where she was pronounced dead shortly after arrival,” the autopsy report read, per Page Six. The bullet “perforated through the right side of the scalp and entered the skull through an entrance-type gunshot wound.”

Naomi Judd had a history of battling mental and physical health ailments such as bipolar disorder, depression, anxiety, hypothyroidism, chronic idiopathic pneumonitis, and more, according to the report. She previously revealed her health struggles to the public, including in her book, River of Time: My Descent into Depression and How I Emerged with Hope.

“She obviously was suffering, and, as such, her days up until that moment were hurtful to her,” Ashley Judd said during an interview with ABC’s Good Morning America.

At the time of Naomi Judd’s death, she apparently had multiple drugs in her body, such as drugs to treat depression and insomnia.

Recently, members of the Judd family filed a court petition to keep documents about Naomi Judd’s death hidden. The country music star is survived by her daughters, husband Larry Strickland, and her grandchildren, among others.
